At what point does reconditioning stop and healthy athleticism begin?

Yesterday’s blog described our basic reconditioning regimen that is designed to take a chronic pain patient and return them to the activities of daily living. Some of our more ambitious patients, however, are not satisfied at this level. They are advised to increase the speed slowly (one tenth of a mile per hour per week) to 3 miles per hour and the length of the walk by one minute per week until they reach 30 minutes on the treadmill. At this point I tell them that they are doing more than the average American. If they want to go further with their exercise regimen, I wish them luck, but tell them that they are now in the realm of athleticism, not medical rehabilitation.
