The third phase of treatment involves exercise therapy to restore strength and muscular balance to the lower back and legs. At Schlesinger Pain Centers we believe that the patient should be pain free before starting this phase of treatment. Like the second phase of therapy designed to restore suppleness we believe that the third phase of strength training never really ends. As with suppleness as strength increases therapy shifts from medical physical therapy venues to gymnasia. Patients making this transition are encouraged to use the services of a reputable physical trainer, preferably one who has additional training in physical rehabilitation. Most of our patients lack a sufficient familiarity with exercise equipment and routines to get the maximum benefit from the therapy and in addition at this age and in these patients, injuries from badly planned or practice exercise can be a serious problem. A word about balanced exercise is in order. Most patients and indeed most athletes have some parts of the training regimen that they like more than others. It is important to remember that many of the patients got into trouble at least in part because the muscular forces acting on the spine had become unbalanced. We certainly do not want to create any new imbalances. Furthermore, I caution patients that as they proceed with strength training it is important to keep up with their suppleness work. In general I suggest adding 1 hour a week to the yoga work for every hour added in the gym. In the upcoming blogs we will look and some of the individual exercises that have proven helpful to our patients.
